Recently i was working as morning shift for four days continually. So i were too tired to update my blog. Wahaha.... As u knew, i loved to sleep.Hee
Oh yeah, i got new things to share with u guys from what i learnt at my attachment area:
1. to wear gloves when u serve the antineoplatic drugs and try not to crush the drugs, this is to protect ur heatlh and the other ppl's safety.
2. for standard precaution to those who have hiv, and throw the things that u have been used to that patient into the biohazard bag, not the normal rubbish bin.
3. after the OGD procedure, the dr will check the HP. If the HP is possitve then dr have to order 2 type of IV antibiotics and 1 losec, it call 'tripple therapy'
4. those who go for colonoscopy, dr have to reset the IV plug as sedative will be given to patient through IV and it help to make patient relax during procedure. Pre-bowel movement likes laxative need to be given one day before procedure.
5. to inform dr when the lab result is abnormal such as the Hb level, potassium level, cardiac enzyme level. As it will cause serious consequance when nil action or meds ordered.
6. dr will check the PT/INR to evaluate the ability of blood to clot properly,they can be used to assess both bleeding and clotting tendencies. Usually dr will check the PT/INR (normal range:2.0-3.0)for those who have heart attacks,strokes,deep vein thrombosis(DVT),those who are taking anti-coagulant drug(eg,warfarin),bleeding disorder or to ensure clotting ability before surgery.Then dr will order the s/c clexane for the pt. The dosage of clexane is depend on the dr's order.
7. dr will order amylase( a blood test)as well as lipase test when they suspect pt has pancreatitis or CA pancreas.
8. for the OGTT( oral glucose tolerance test), patient have to nil by mouth 12 MN then the fasting blood taking at 0800hr, and let patient to drink 75mg glucose of water. To take blood again after two hours.This is to track the glucose level over time.
9. to omit the insulin or DM meds when pt is on NBM even if the hypocount is high.
10. to do pressure bandage on the first day of removal CVP line, it usually done by dr when he or she remove it. After that, the pressure bandage can be taken out after 24 hours.
11. to inform dr when pt is vomitting. It's important. Must let dr to know about it, then dr usually will order anti-emetic meds to the pt.
12. to inform dr to reset IV plug if pt go for MIBI scan. The full term of MIBI scan is vasovuscular stress/rest test. Coffee, milo,tea or milk are no allowed for the pt before the procedure, as it will act as a stimulant. MIBI will be injected through IV and then picture of heart will be taken for the rest part. After that pt can return back to ward to hv lunch. Stress part of MIBI scan is done when pt send to cardiology to do exercise and the picture of heart to be taken. It take about 4 hours for the procedure.
